Student Information

Enter the student's last name.
Include the middle initial here if the student uses one.
Enter the student's first name.
Enter the student ID.
Select the grade level.
Only the sections for the selected grade level are listed.

Required — this photo is printed on the digital gate pass, so the application cannot be submitted without it.
  • Recent colored 2×2 ID photo, in official SISC uniform, plain white background.
  • JPG or PNG files only. PDF, HEIC, and other formats are not accepted.
  • Name the file after the student — last name, first name, as entered above, for example Dela Cruz, Juan.jpg. If your file has a different name, it is renamed automatically on submit and the final name is shown below.
  • Maximum file size: 3 MB.

Gate Pass Details

Tick every gate the student is allowed to use. Selecting all gates is recorded as "Any authorized gate".
Tick at least one exit gate.
Select one dismissal arrangement.
